If your FUTA tax liability for any calendar quarter is over $500 (including any FUTA tax carried forward from an earlier quarter), you must deposit the tax by EFT. When you fill out Form 941, be sure to check the box on the top of the form that corresponds to the quarter reported. The facts are the same as in Example 2, except you elect to use the flat rate method of withholding on the bonus. You then figure withholding based on the daily or miscellaneous period.
